Abstract
Acute Coronary Syndrome is a subset of coronary heart disease characterized with a sudden reduction in blood flow to heart muscle (myocardium) caused by disruption in the coronary blood vessels. Non-communicable diseases, one of which is coronary heart disease, causing of death for almost 70% in the world. This study aims to analyze the cost effectiveness of using aspirin monotherapy and aspirin-clopidogrel combination in patients with acute coronary syndrome at Dr. Moewardi Hospital. This study used descriptive non-experimental method with retrospective data collection design. Data collected were in patients with acute coronary syndrome from January 2023 to October 2024. The number of samples was 74 patients with details of 34 patients using aspirin and 40 patients using aspirin-clopidogrel. Data analyzed with ACER and ICER. Average total direct costs of acute coronary syndrome patients using aspirin with an ACER value of Rp. 6.601.050, while aspirin-clopidogrel with an ACER value of Rp. 10.887.826. ACER analysis shows that aspirin is more cost-effective than the combination of aspirin-clopidogrel in patients with acute coronary syndrome hospitalized at Dr. Moewardi Hospital. Based on ACER value, aspirin showed higher effectiveness with lower cost. The ICER analysis showed that aspirin-clopidogrel therapy requires additional costs of Rp. -267.923. Based on the results of ACER and ICER analysis, aspirin monotherapy is more cost-effective than aspirin-clopidogrel combination therapy for patients with acute coronary syndrome hospitalized at Dr. Moewardi Hospital.
