Abstract
According to Bird and Germain, unsafe action, unsafe condition, and near miss are things that should be learned and prevented so that work accidents do not occur that can cause a number of losses. This study aims to explore the factors that cause near miss in hot work activities at PT Trans-Pacific Petrochemical Indotama Tuban. This research is a descriptive qualitative research, through interviews with ten informants in the mechanical engineering division and field observation at PT Trans-Pacific Petrochemical Indotama Tuban. The results of the research are based on interviews with informants at PT. Trans-Pacific Petrochemical Indotama Tuban, that there are several workers who do not clearly understand the occurrence of near miss and there has been no reporting of near miss to workers who carry out hot work activities. Conclusion The workforce at PT. Trans-Pacific Petrochemical Indotama Tuban, Labor does not understand near miss and has not reported it officially. Risk factors include unsafe actions such as rushing, lack of focus, and not wearing PPE, as well as unsafe conditions such as narrow spaces, hydrocarbon leaks, and excessive temperatures. The suggestion in this study is that companies should hold socialization about near miss and the importance of reporting, especially in hot work activities, to prevent work accidents. In addition, it is necessary to conduct safety talks and toolbox meetings regularly to increase workers' awareness of safety and OSH procedures.
