Indonesia has the largest Muslim population in the world, with over 207 million Muslims, reflecting a high demand for halal services, including in the health sector. In this context, the implementation of Sharia Pharmaceutical Services represents a strategic innovation to ensure that pharmaceutical practices align with Islamic law. This study analyzes the implementation and quality of Sharia Pharmaceutical Services at RS PKU Muhammadiyah Temanggung. A descriptive qualitative method was used with purposive sampling, involving pharmacists, pharmacy technicians, members of the Therapeutic Committee, and the Sharia Committee. Data collection methods included in-depth interviews, document reviews, observations, and evaluation checklists based on MUKISI and DSN-MUI standards. Thematic analysis was employed, and service quality was assessed using Likert scales, binary scoring, and percentage conversions. The findings show that Sharia Pharmaceutical Services are well-integrated into hospital operations and policies. This includes Sharia-based drug selection, documentation of haram and najis substances, informed consent aligned with Islamic ethics, religious messages in pharmaceutical documentation, and routine Islamic education for staff. Performance indicators revealed high levels of implementation: formulary compliance (90%), completeness of documents (83.33%), and implementation by personnel (98.19%). These results demonstrate that Sharia Pharmaceutical Services are not merely symbolic but are embedded in management systems, SOPs, and human resource development. The collaboration between the Sharia and Therapeutic Committees has enhanced regulatory compliance and service quality. RS PKU Muhammadiyah Temanggung exemplifies how Islamic values can be harmonized with modern healthcare for Sharia-based healthcare governance both nationally and globally.
